HealthPro will provide our pharmacists a new employee referral bonus of fifty dollars ($50) to any employee, except management, who refers a potential employee candidate for hire.
The referring employee must complete this form before the candidate is contacted by our staff, the new candidate must be hired by HealthPro, and the new candidate must complete one work assignment.
A maximum of ten bonuses per calendar year will be paid to each eligible employee (up to $500).
By submitting this form, you understand that you, the employee, are referring the below candidate for hire. The below candidate will be contacted by our staff. After the above conditions are met, you, the employee will receive a $50 bonus.
